What do I need to bring to my first appiontment?

Call Changing Minds Psychiatry at 702-405-8088 or (833)575-4012 to schedule my first appointment.
- Provide my insurance numbers to the staff assisting me with scheduling. Including any secondary insurance.
- For cash pay patients you are not required to have insurance on file.
Complete all new patient paperwork via the patient portal prior to my first appointment.
Upload photos of the front and back of my insurance card(s) to the patient portal.
Upload copies of any applicable Court Ordered Custody Agreements, Power of Attorney, Conservatorship, Guardianship paperwork, and/or Foster Placement Letters to the patient portal.
Bring my physical drivers license/Identification card to the first appointment.
On the day of my appointment arrive 30 minutes early, checkin at the Kiosk by providing my last name and date of birth and verify that all the new patient paperwork was received.
Understand that not having insurance on file will result in paying the cash pay rate for your appointment. Not having your new patient paperwork completed within 24 hours of your appointment runs the risk of your appointment being cancelled.
