Supporting Access to Care
At Changing Minds Psychiatry, meeting patients where they are is one of our core values. To better support access to care for patients residing in group homes, care facilities or those represented by legal or public guardians, conservators, or powers of attorney, we have developed a secure electronic consent form.

All consent forms must be submitted within 24 hours of the patient’s scheduled appointment.
Failure to provide completed consent forms from the authorized representative may result in prescriptions not being sent to the pharmacy.
For additional required documentation—such as Proof of Guardianship, Court Orders, or Individual Service Plans—please email them to Records@changingmindspsych.com.
