Supporting Access to Care
At Changing Minds Psychiatry, meeting patients where they are is one of our core values. To better support access to care for patients residing in DFS/DCFS custody, group homes, care facilities or those represented by legal or public guardians, conservators, or powers of attorney, we have developed a secure electronic consent form.
Please use the link below to complete the online Adobe form.
Note: Include the names of all individuals/agencies who need to have access to coordinating this patient’s care to the consent forms.
All consent forms must be submitted in order to schedule a new patient appointment and updated consents must be received in order to keep previously scheduled appointments.
Failure to provide completed consent forms from the authorized representative may result in prescriptions not being filled.
Additional required documentation—such as Proof of Guardianship, Placement letters, Court Orders, or Individual Service Plans—please email them to Records@changingmindspsych.com.
