Walk In Only Status Patients
" For Patient's that are receiving medication management and have no showed/late cancelled 3 appointments with in a calendar year or cancelled consecutively will be placed on Walk In Only Status. These patient's must have received a warning letter after the second missed appointment.
Procedure:
Patient missed two appointments whether that be No Show or a Consecutive Cancelation the provider and/or staff should be notifying the Case Manager(s) via the Distribution List.
The Case Manager(s) will create an Attendance Warning Letter and upload to the patient's chart as patient viewable The Case Manager(s) will send the patient a portal message telling them where they can review the notice.
Patient missed a third appointment, The Case Manager(s) will be notified that they are on Walk-In Only Status via a letter and a patient portal message.
Adding the risk factor "WALK IN PATIENT".
After the patient attends their appointment successfully change the risk factor to "WALK IN PATIENT (HISTORIC)"
Patient misses a fourth appointment the patient is permanently on Walk-In Only Status via letter and a patient portal message until the end of the calendar year. The Case manager needs to update the Risk Factors in the patient's chart.
Update the risk factor to "WALK IN PATIENT (YEAR NOTICE).
