Policy Title: Policy for Gender Affirming Treatment Clearance 

Purpose:

This policy outlines the procedures and guidelines for mental health providers at Changing Minds Psychiatry who are responsible for writing clearance letters for transgender reassignment surgery (gender-affirming surgery) and hormone therapy. The policy ensures that the process is thorough, respectful, and in compliance with professional standards and legal requirements.

Policy Statement:

Changing Minds Psychiatry is committed to supporting the health and well-being of transgender individuals by providing competent, respectful, and evidence-based mental health services. This policy ensures that clearance letters for gender-affirming surgery and hormone therapy are issued based on comprehensive evaluations and in accordance with established guidelines and standards of care.

Procedures:

  1. Initial Assessment

    1. Comprehensive Evaluation: Conduct a comprehensive evaluation of the patient’s mental health, gender dysphoria, and readiness for surgery or hormone therapy. This should include:

      1. A detailed psychosocial assessment

      2. An evaluation of the patient’s understanding of the surgery, including risks and benefits

      3. An assessment of the patient’s mental health history and current mental health status

    2. Informed Consent: Ensure that the patient fully understands the process and purpose of the evaluation, including the potential outcomes and implications of the clearance letter.

  2. Criteria for Clearance

    1. Standards of Care: Follow the standards set forth by the World Professional Association for Transgender Health (WPATH) or other relevant professional guidelines, which typically include:

      1. Persistent, well-documented gender dysphoria

      2. Capacity to make a fully informed decision and to consent for treatment

      3. Age of majority (in most jurisdictions, this is 18 years or older)

      4. If significant medical or mental health concerns are present, they must be reasonably well controlled

    2. Duration of Care: Generally, the patient should have been under the care of a mental health professional for a specified period, often recommended as a minimum of 12 months. 

  3. Writing the Clearance Letter

    1. Content Requirements: The clearance letter should include:

      1. The mental health professional’s credentials and relationship to the patient

      2. A statement that the patient has been assessed according to WPATH or other relevant guidelines

      3. Confirmation of the patient’s diagnosis of gender dysphoria

      4. A summary of the patient’s mental health history and current mental health status

      5. A statement of the patient’s capacity to consent to surgery or hormone therapy

      6. An assessment of the patient’s understanding of the surgery or hormone therapy, including risks and benefits

      7. An endorsement of the surgery or hormone therapy as a medically necessary and appropriate step for the patient

    2. Professional Tone: The letter should be written in a professional and respectful tone, reflecting the seriousness of the evaluation and the importance of the surgery or hormone therapy for the patient’s well-being.

  4. Documentation and Record Keeping

    1. Secure Records: Maintain secure and confidential records of all evaluations and clearance letters in the patient’s file.

    2. Patient Access: Patients should have access to their records in accordance with HIPAA and state regulations.

  5. Training and Competency

    1. Provider Training: Mental health providers must receive training in transgender health, gender dysphoria, and the standards of care for transgender individuals.
