What do I need to bring to my first appiontment?

  1. Call Changing Minds Psychiatry at 702-405-8088 to schedule my first appointment.

    • Provide my insurance numbers to the staff assisting me with scheduling. Understand that not having insurance on file within 24 hours of scheduling will result in my appointment being cancelled.

    • For cash pay patients you are not required to have insurance on file.

  2. Complete all new patient paperwork via the patient portal prior to my first appointment.

  3. Upload photos of the front and back of my insurance card to the patient portal.

  4. Bring my physical drivers license/Identification card to the first appointment.

  5. On the day of my appointment arrive 15 minutes early, checkin at the Kiosk by providing my last name and date of birth and verify that all the new patient paperwork was received.
